RELEASE AND WAIVER OF LIABILITY
Escape Dance Academy (EDA) — Private Lesson Waiver
In consideration of being permitted to participate in dance instruction, private lessons, classes, rehearsals, performances, and any related activities (collectively, the "Activities") provided by Escape Dance Academy ("EDA"), its instructors, owners, employees, contractors, and agents (collectively, "Released Parties"), I, the undersigned participant — or parent/legal guardian on behalf of a minor participant — agree as follows:
1. Assumption of Risk. I understand that dance instruction involves inherent risks including, but not limited to: muscle strains, sprains, fractures, bruises, falls, collisions with floors, equipment, mirrors, or other participants, and other serious injuries. I voluntarily assume all such risks, known and unknown, and accept full personal responsibility for any injury, damage, or loss that may occur during the Activities or while on EDA premises.
2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any injury, illness, loss, or damage to person or property sustained while participating in the Activities or while present on EDA premises, whether caused by the negligence of the Released Parties or otherwise — to the fullest extent permitted by Ohio law.
3. Medical Treatment. I authorize EDA staff to seek emergency medical attention on my (or my child's) behalf if needed, and I accept financial responsibility for any such treatment.
4. Photo/Video Release. I grant EDA permission to use photographs and video taken during Activities for promotional and instructional purposes, unless I notify EDA in writing otherwise.
5. Policies. I agree to follow all EDA policies, dress codes, and instructor directions. I understand that failure to do so may result in removal from the program without refund.
6. Acknowledgment. I have read this waiver carefully, understand its contents, and sign it freely and voluntarily. I understand this is a release of legal rights and that this waiver remains in effect for 365 days from the signing date.
